14Highlights of EAP 2023PresenterFenoldopam is a splanchnic vasodilator which is used in prevention and functional recovery of patients affected by AKI. It can be used as antihypertensive drug in hypertensive urgencies and emergencies in children, however there is no clear evidence in the literature. Sodium nitroprusside is generally the first choice of treatment in patients with %u2191hypotension, renal toxicity.Dr. Nicola Bertazza Partigiani and his team conducted a study, which evaluated the safety and effectiveness offenoldopam in pediatric population, especially inchildren affected by kidney disease. Also, theyevaluated its effectiveness in reduction of renalresistive index. This is an unpublished data that was discussed by Dr. Partigiani. Children admitted to the PICU and/or pediatric nephrology of Padua Children%u2019s Hospital with hypertensive urgencies and emergencies (systolic or diastolic BP >95% + 30 mmHg were included (n=74). The Renal Resistive Index was measured before and after 24%u201372 hours of treatment. It was observed that there was no reduction of RI (0.77 vs. 0.74; p=0.27). Thus, in conclusion, fenoldopam is an effective and safe drug in the initial management of hypertensive urgencies-emergencies in children; with an initial dose of 0.5 mcg/kg/min; and has a low incidence of hypotension and other adverse events.Figure 1: Reduction in >10% or >20% BP at 8 h.
